➢ SS Number
➢ New registrant::
• Single: E-1 form and Birth Certificate (NSO/Certified True Copy by Local Civil Registry).
• Married: E-1 form, Birth Certificate and Marriage Contract.
Original and xerox copy for verification. Only xerox copy will be submitted.
Prepare active email address and mobile number.

➢ ID Capture:
• UMID form and 2 valid ID cards
Lost /Change ID: UMID form, 2 valid ID’s, Affidavit of Loss and R-6 form for payment of 300

➢ Annual Confirmation of Pensioners:
• ACOP form, 2 valid ID’s
